REGISTRATION FORM

.Complete  online application and submit with $75.00 (non-refundable) application fee.  Submit a copy of your child's Certificate of Immunizations, Social Security card and Certified Birth Certificate (not mother's copy) .  Email : wofcastudentsontherise@gmail.com  or  Fax: (901)744-4069 documents to Admissions.  Admissions will email a records release form to you, if applicable, to complete and return by email or fax.  All  new students (1st-12) Academic records will be reviewed.  Admissions will  call  to schedule an appointment/tour for all new students.   All returning students must make sure vital records are current.
